Title: Licensed Practical Nurse (LPN)

 

Location:  On-site Various Locations

 

Job Summary:

The Licensed Practical Nurse (LPN) is responsible to assist with nursing services for children in an on-site children’s residential facility. Responsible to assist the Registered Nurse (RN) in the assessment, intake and daily medical needs of the children during their stay at the facility. They ensure contractual requirements and client expectations are met at the on-site residential facility. 

 

Reports to:

·      RN or Charge Nurse.

 

Principle Accountabilities:

·      Assists with routine and daily medical needs of the children.

·      Refers children to site physician, physician assistant, or nurse practitioner as needed for medical care and assists with transfers to a specialist or hospital.

·      Refers children deemed at risk of harm to self or others to site mental health professional.

·      The Licensed Practical Nurse (LPN) maintains medical case file with appropriate documentation in accordance with policies and procedures.

·      Assists with operation of medical clinic.

·      Works closely with case managers, mental health providers, and licensed clinicians.

·      Provides group education on various safety and disease prevention topics.

·      Ensures proper plan of care is implemented. 

·      Other duties as assigned by supervisor.

 

Other Duties:

·      Performs duties in a safe manner.  Follows the corporate safety policy.  Participates and supports safety meetings, training and goals.  Ensures safe operating conditions within area of responsibility. Encourages co-workers to work safely.  Identifies “close calls” and/or safety concerns to supervisory personnel.  Maintains a clean and orderly work area.

·      Assists in active implementation of company initiatives to ensure compliance with OSHA VPP, ISO, JCAHO, AAAHC, and other mandated regulations/standards.

 

Experience & Education:

·      Must be able to multi-task, be detail-oriented, be organized and have excellent verbal and communication skills.

·      Licensed in any state.

·      1 year of experience with pediatrics or adolescent medicine.

·      Bilingual in Spanish and English. (Read, Write, Speak)

·      The Licensed Practical Nurse (LPN) must be First Aid/CPR certified.

·      Clean criminal background check.

·      Clean Child Abuse and Neglect (CAN) or child protective services check.

 

Preferred Qualifications:

·      1 year of experience in a residential facility.

 

Physical Requirements:

·      Must be able to ascend/descend stairs.

·      Must be able to lift up to 50 pounds.

·      Must be able to withstand changing environmental conditions with weather (rain, heat, lightning, and winds)

·      Must be able to withstand and manipulate through construction areas, sports fields, etc.

·      The Licensed Practical Nurse (LPN) Must be able to stand; particular for sustained periods of time.

·      Must be able to receive and understand detailed information through oral communication. 

·      Must be able to sit for more than 8 hours per day

·      Must be willing to work over time if necessary

·      Must be able to routinely grasp or handle objects, use finger dexterity, bend elbows/knees and reach above and below shoulders

·      Must be able to read and interpret handwritten and typewritten print

·      Must be able to communicate by voice and detect sound by ear

·      Must be able to pass a criminal background investigation

·      Must complete Urine Drug Screen
