This full-time nurse practitioner (NP)/physician assistant (PA) role will include patient care rounding in post-acute care a skilled nursing facility located in Seattle, Washington. Job Duties:- You will be a part of a collaborative team environment that includes a physician and NP/PA providing primary care in a post-acute setting
- Care includes but is not limited to admissions, progress follow up, acute follow up, and discharge
- You will also work with the facility to transfer patients to the level and site of care most appropriate for their medical needs upon discharge including returning to home, independent living centers, assisted living, and/or memory care
- Our clinicians work closely with hospice, palliative care, and home care agencies to ensure a safe transition home
- Our clinicians are also responsible for timely documentation and billing that meets Medicare guidelines through TeamHealth's EMR
- Rounding will take place Monday through Friday with flexible daily hours

Qualifications:
- New grads accepted
- Board certified NP/PA
- Active Washington license or application in process
- Active and unrestricted
