Join Providence Community Health Napa Valley, as a Per Diem nurse practitioner as part of an exciting new GUIDE program providing all-inclusive care to elderly patients across the Napa region. GUIDE offers enhanced services for dementia care to improve the quality of life for people living with dementia and to enhance support to their caregivers. This is an outpatient role where you can expect to do assessments for primary care, clinic visits, coordinate referrals to specialty appointments, be a liaison to primary care physicians in our new GUIDE program, as well as provide as-needed back-up coverage to the PCP. As a Per Diem role, this is a 2-day per week schedule to start, with around 5 to 6 shifts per month.

• Per Diem role with 8-hour weekday shifts
• 1- 2-days per week to start with additional call opportunities
• Outpatient care only
• Focus on providing comprehensive care to elderly patients across the Napa Valley region
• Must be comfortable working autonomously
• New graduates with geriatric experience are welcome to apply
• Compensation is between $80.40 and $108.83 per hour
• The posted salary reflects the starting range of total compensation and may include productivity bonuses, incentives related to quality and performance, hospital/ED call stipends, extra shift incentives, and other forms of cash compensation as applicable to the position. In addition, providers typically receive a CME allowance and other benefits offered by their medical group employer. Please note that this salary range is provided in accordance with State law and is subject to variation due to the factors noted above.
