Lutheran Health Network in Warsaw, IN is seeking an experienced Nurse Practitioner and/or Physician Assistant to join our established Medical Group in Syracuse, Indiana.
Primary duties and requirements:
- Patients from newborns to geriatrics, providing multifaceted healthcare services within a hybrid Family Medicine Practice by appointments and walk-in, in addition to telemedicine appointments
- This position is a Full time, 40 hour workweek opportunity
- 1 - 2 years’ experience FNP or PA required
- Excellent interpersonal skills
- Telemedicine experience and/or willingness learn & incorporate into practice
- Creative problem solver/critical thinker
- Open to flexibility and versatility in accepting different Practice assignments
- Team player, adaptability in working with different staff easily
Kosciusko Community Hospital (KCH)
Located on a 30 acre medical campus, KCH is a 72 bed, medium rural size facility, with all private rooms, offering general and surgical services. Women's medical services extended include labor, delivery, nursery, breastfeeding support, eLearning Childbirth Education and mammography.

Key Responsibilities:
Perform evaluations/examinations and problem identification of patient. Administer or order and interpret lab and other diagnostic tests. Conduct routine physicals. Provide primary health care with emphasis on preventative care. Prescribe appropriate therapeutic interventions or medication with physician approval. Assist in minor surgeries. Instruct/counsel patient, or parent, on prescribed medications and health maintenance. Obtain, compile and record patient medical data electronically (Cerner), including health history, progress notes and results of physical exams, findings. Make appropriate referrals when needed.
License Requirements:
- Valid Indiana State Medical License
- Master's Degree in Nursing (Nurse Practitioner or Physician Assistant)
- Board Certified
