Fairview Health Services is seeking a Nurse Practitioner (NP) to provide long-term practice coverage for our primary care clinics with Fairview Clinics - Float Pool.
The NP, in collaboration with physicians and part of an interdisciplinary team, engages in independent decision making about health care needs of the clinic patients. Assess patients, order and interpret diagnostic tests, make diagnoses, and initiate and manage treatment plans including prescribing medications. While maintaining a focus on health promotion and disease prevention, the NP will manage chronic disease conditions ensuring continuity of care for individuals in the communities served. As part of our participation in the Pioneer Accountable Care Organization (ACO), the practitioners use a team approach in managing clinical care.
