Fairview Health Services is seeking 2 (.6 FTE) Nurse Practitioners (NPs) to join a primary care practice at Fairview Clinics - Rogers to provide same day care and assist with growth of the practice The NP's will collaborate closely with the primary providers on site to offer patients a seamless transition from same day appointments to establishing care and will assist with management of chronic health and preventative medicine.

- Hours of the position are 10:00am-7:00pm 3 days per week.
As part of our participation in the Pioneer Accountable Care Organization (ACO), the physicians use a team approach in managing clinical care. The site is using a model of care, Care Model Innovation, for their patients which actively looks at and changes clinical processes to maximize efficiency, resources, and clinical care.
Position Qualifications:
A qualified candidate will be a graduate of an accredited school of nursing and formal Family Nurse Practitioner training program. Have a current Minnesota APRN license and be approved or eligible by the Minnesota Board of Nursing for prescription writing privileges. Be eligible or certified through ANCC or AANP and meet threshold criteria/qualifications for Credentialing and Privileges. Ability to obtain and/or maintain DEA certification for the State of Minnesota.
Preferred Qualifications:
Previous NP experience is required.
