In Presidio, TX: Texas Association of Community Health Centers is seeking a full-time Nurse Practitioner to deliver quality medical care to patients of the Center and functions as a member of the multi-disciplinary health care team within the scope of his or her medical practice.
Job Overview:
- Provide complete, comprehensive outpatient care within his/her scope for patients, including health care maintenance, examinations, referrals, and treatment of medical conditions, to include follow-up on all patients and/or any problems detected
- Medically manage acute and chronic illness, including acute complication and routine follow-up. Develop a plan of care, coordinate care and determine the need for case conference
- Make appropriate referrals to both internal disciplines of the agency and /or external agencies for additional services required, which also include health care management, mammograms, well women, and well men physicals and follow-ups
- Provide appropriate follow-up with assistance of the nursing staff to include telephone/written correspondence. Consult with medical and nursing staff as indicated
- Responsible for accurate, coding of charts, timely documentation in the patient’s medical record, client contracts, case management, client’s plan of care, health care management, referrals, and follow-ups
- Provide leadership and assist in efforts to improve the efficiency of the medical clinics
- Participate in various administrative/clinical staff meeting, but not limited to, Continuous Quality Improvement committee, clinic meetings, PEER Review, general staff meetings, case conference/management, and /or administrative meetings as required

Qualifications:
- License to practice as a Family Nurse Practitioner in the State of Texas required
- Annual continuing medical education as required by Board specialty
- Board certification required
- Current Cardiopulmonary Resuscitation (CPR) for Healthcare Professionals Certification
- Minimum of 3 years experience preferred
- Knowledge of protocols in general and an ability to quickly gain specific knowledge of the Center’s current operational protocols
