Seeking a part-time (0.6 FTE), board-certified nurse practitioner with experience in geriatrics, adult primary care or family medicine. The practitioner in this position will serve as a clinical float, providing coverage to ElderPlace locations in the Portland metro area. This is an excellent opportunity to serve a geriatric population within an interdisciplinary team made-up of physicians, nurse practitioners, therapists, social workers and care coordinators. Providence ElderPlace is a Program of All-inclusive Care for the Elderly (PACE), and is the first program of its kind in Oregon. With eight conveniently located centers in the Portland area, Providence ElderPlace provides and coordinates all necessary care services for seniors to remain active within their communities. The PACE model offers medical care, physical and occupational therapy, lab tests and procedures, home care services and more.
