Providence Health & Services is seeking an excellent on-call nurse practitioner to join an innovative, comprehensive care and social services program, Providence ElderPlace. We are seeking a primary care clinician to see participants in a variety of community-based settings including clinic, assisted living, memory care, and private residences. This is an outstanding opportunity to join an interdisciplinary practice model providing older adults and their families the comfort of a single, coordinated program of health care, housing, and social services. Current opportunity is at our Seaside location on Oregon’s beautiful North Coast, near our 25-bed critical access facility, Providence Seaside Hospital.

Where You’ll Work
Providence ElderPlace is a Program of All-inclusive Care for the Elderly (PACE), and is the first program of its kind in Oregon. With eight conveniently located centers in the Portland area, Providence ElderPlace provides and coordinates all necessary care services for seniors to remain active within their communities. The PACE model offers medical care, physical and occupational therapy, lab tests and procedures, home care services and more.
