• Salary: Range is 105,000-135,000 (Experienced provider with bonus can make $150-$160k)
• Annual clinical performance bonus (last year NP’s with less than 5 years experience made 15k annually and more seasoned with 5+ years exp. made 25k bonus)
• Reimbursement for Mileage and Toll fees
Role: Transitional Health - (Home Health)
• Focused on Post Discharge visits Includes
-Medication reconciliation
-Social service follow ups
-Follow up care in general with follow up care coordination.
• Seeing 6-7 patients a day
Will be supported by the Medical Director
Qualifications
Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
Ideally 5+ years’ experience in either primary care in clinic or Home setting with geriatric population
Active, non-probationary Pennsylvania Nurse Practitioner license
Active DEA license
US Work Authorization
Post discharge visits/ transitional care experience a plus
National certification in at least one of the following specialties:
Family Nurse Practitioner
Adult-Gerontology Primary Care Nurse Practitioner
Adult Nurse Practitioner
Gerontological Nurse Practitioner
