The hospital medicine advanced practice clinician (APC) provides high-quality, patient-centered medical care to hospitalized adult patients in collaboration with hospitalist physicians and the interdisciplinary care team. The APC is responsible for evaluation, diagnosis, treatment, and ongoing management of acute and chronic medical conditions in the inpatient setting, ensuring safe, efficient, and evidence-based care.
About the Role- Perform comprehensive history and physical examinations for admitted patients
- Manage acute and chronic medical conditions in hospitalized adults
- Develop, implement, and adjust treatment plans in collaboration with supervising physicians
- Order, interpret, and act on diagnostic tests (labs, imaging, EKGs, etc.)
- Perform procedures as credentialed
- Respond to clinical deterioration, rapid responses, and medical emergencies as required
- Participate in admissions, daily rounding, transfers, and discharges
- Conduct medication reconciliation and ensure safe prescribing practices
- Collaborate with physicians, nursing staff, case management, pharmacy, therapy services, and consultants
- Communicate effectively with patients and families regarding diagnoses, treatment plans, and goals of care
- Participate in multidisciplinary rounds and discharge planning
- Provide thorough handoffs during transitions of care
Competitive compensation with an estimated base salary range of $75 to $85 hourly with the opportunity to earn incentives.
