Fairview Health Services is seeking a Nurse
Practitioner to join a primary care practice at Fairview Clinics - Burnsville.
As part of our participation in the Pioneer Accountable Care
Organization (ACO), the providers use a team approach in managing clinical
care. The site is using a model of care, Care Model Innovation, for their
patients which actively looks at and changes clinical processes to maximize
efficiency, resources, and clinical care.
Practice Details:
•Join a 9 Practitioner practice.
•Provide outpatient care.
•Patient encounters average 18-20 patients per day.
•Onsite services include: pharmacy, diabetic education, lab,
medication therapy management, nutrition education, psych, geriatric, physical
therapy, radiology (digital) including: cath lab, CT, DEXA, Mammography, MRI,
X-ray, and ultrasound. Pain management services nearby.
•Onsite specialties include: Internal Medicine, Pediatrics,
and Obstetrics and Gynecology
•We have a fully integrated Electronic Medical Record (EPIC).
•Physicians work an expectation of 36 patient contact hours per
week.
•Patients are admitted to the hospitalist service at Fairview
Burnsville Hospital.
•Opportunity for physicians to perform procedures.
•Openings: 1.0 FTE
•Call Frequency: 1:10 (low intensity: phone consultation only with
first tier nurse triage)

Position Qualification:
Graduate of a school or nursing and
accredited Nurse Practitioner training program. Licensed or
ability to obtain APRN license in the state of MN and Board
Certified/Board Eligible through the AANP or ANCC. Ability to obtain and
/or maintain DEA certification for the State of Minnesota. Meet threshold
criteria/qualifications for Credentialing and Privileges.
