TrinityCare Hospice is seeking a full-time Palliative Care nurse practitioner to support its outpatient care program in the South Bay/Los Angeles area with some Orange County coverage. This position is fairly autonomous, though you will be supported by another NP and an MD as you conduct work through a mix of home-visits and virtual care. - Monday to Friday schedule, 8-hour shifts

Providence TrinityCare Hospice is one of the oldest hospice programs in the greater Los Angeles area, with a staff of 180 professionals who provide compassionate end-of-life care to more than 30,000 patients and families. The facility consults and partners with all six Providence medical centers in Southern California.
