Palliative Care Nurse Practitioner (APRN) – Norwalk Hospital, Norwalk, Connecticut (One hour from New York City)

Norwalk Hospital is seeking a Palliative Care APRN for inpatient palliative care services at Norwalk Hospital in Norwalk, CT, located in Fairfield County, about one hour northeast of New York City. This position works with our interdisciplinary team in a growing Palliative Care Department to provide Palliative Care Services mainly in the inpatient setting.

This is an employed position with Western Connecticut Medical Group. We offer a comprehensive compensation package, including competitive salary, generous benefits, and allowances for CMEs and Dues.

Western Connecticut Health Network includes Norwalk, Danbury, and New Milford Hospitals in addition to our Medical Group, which has over 70 outpatient practice locations.  Norwalk Hospital has 366 beds and is also a Level II Trauma Center with approximately 50,000 ED visits per year. As a teaching hospital, we are associated with the Vermont College of Medicine, Yale University School of Medicine, Ross University School of Medicine and the American University of the Caribbean School of Medicine. Our Medical Group includes 400+ physicians and 150+ advanced practice providers, serving a geographically broad region throughout Western Connecticut and eastern New York.

 Job Summary:

The primary function of the APRN is to provide medical care for patients in collaboration with the physician. The APRN is responsible and accountable for the clinical management of adult patients appropriate for palliative care.

The APRN is responsible for carrying a patient case load in collaboration with the palliative care team. The APRN is expected to assess/refer patients for both the physical and psychosocial needs of the patient. The position will report to Western CT Medical Group Director of Palliative Care.

Job Requirements:

  • This position requires a minimum formal education of Master Degree, State of CT RN and APRN license, and Federal DEA and CT Controlled Substance Registration.

  • At least 1 year experience as an APRN, preferably in Palliative Care, Hospice Care, and/or Oncology settings is required.

  • Excellent communication, clinical and interpersonal skills to facilitate patient care between hospitalists, private physicians, referral and collaboration amongst other hospital staff.

  • Able to work well with a large multidisciplinary team.

  • Able to work confidently and independently.

  • Board Certification in Advanced Certification in Hospice and Palliative Nursing (ACHPN) required within 2 years of hire for APRN
