Providence Connections Palliative Care is seeking a board-certified nurse practitioner to join their inpatient team in Oregon City, Oregon. This well-established, trans-disciplinary team provides a coordinated program to patients and their families for the management of chronic conditions or serious, life-limiting illness. The ideal candidate must have previous Palliative Care are experience. Primary location will be Providence Willamette Falls Medical Center. Connections Palliative Care is part of a growing integrated Home and Community Services program which collaborates with Hospice, Home Health, Geriatrics as well as hospital-based practitioners to care for patients and their families.
- 0.9 FTE position (considered full-time for benefit purposes); Monday to Friday
- Collaborate care with physicians, nurses, social workers, pharmacists, chaplains
- Minimum of 1 year of post-graduate clinical experience in Palliative Care, Hospice, Geriatrics, and/or Oncology Care is preferred
