Role:

• Provide primary care in a skilled nursing facilities in Creswell, Oregon, 1 day a week
• Provide primary care treating a diverse patient population, manage chronic conditions, and collaborate with a multidisciplinary team
• Build long-term relationships with patients for improved care continuity

Responsibilities:

• Deliver primary care for short-term and long-term patients
• Manage chronic diseases, diagnose conditions, and create care plans
• Order and interpret lab results, prescribe medications, and consult with other healthcare providers
• Educate patients and their caregivers, and promote healthy lifestyle changes

Requirements:

• Must have a minimum of 6 months primary care experience
• Post-acute care experience is a plus
• Active and unrestricted Oregon NP license and DEA, or application in progress

Compensation and Benefits:
Our compensation is a productivity-based model (Fee for Service) that offers you control to maximize your income potential. Expected compensation is estimated salary range of $24,000 and $29,000 annually with no cap productivity income potential.
