Fairview Health Services is seeking a Physician Assistant (PA) to provide long-term practice coverage for our primary care clinics with Fairview Clinics - Float Pool.
The PA, in collaboration with physicians and part of an interdisciplinary team, engages in independent decision making about health care needs of the clinic patients. Assess patients, order and interpret diagnostic tests, make diagnoses, and initiate and manage treatment plans including prescribing medications. While maintaining a focus on health promotion and disease prevention, the NP or PA will manage chronic disease conditions ensuring continuity of care for individuals in the communities served. The NP or PA may also perform minor surgical procedures within their scope and realm of the collaboration agreement with a physician.
As part of our participation in the Pioneer Accountable Care Organization (ACO), the practitioners use a team approach in managing clinical care. The site is using a model of care, Care Model Innovation, for their patients which actively looks at and changes clinical processes to maximize efficiency, resources, and clinical care.
Practice Details:
•Work in a primary care clinic setting at multiple locations
•Schedule: Monday- Friday (36 patient contact hours)
•Travel credit is provided
•Fully integrated Electronic Medical Record (EPIC).
Position Qualifications:
A qualified candidate will be a graduate of an accredited Physician Assistant program. Have a current PA license with prescriptive authority through the Minnesota Board of Medical Practice. Be eligible or certified NCCPA and meet threshold criteria/qualifications for Credentialing and Privileges. Ability to obtain and/or maintain DEA certification for the State of Minnesota.
Preferred Qualifications:
Previous primary care experience preferred.
