Providence Medical Group’s Phoenix Family Medicine Clinic is seeking a full-time Family Medicine nurse practitioner or physician assistant to join the Phoenix Family Medicine Clinic, a clinic structured as a Medical Home Team Care Model with Providers, Behavioralist, Pharmacist, and a Case Manager. This smaller clinic makes a profound impact in the community of Phoenix, Oregon, where it is focused on engaging a diverse patient population.
