Providence Medical Group (PMG) is seeking an experienced nurse practitioner or physician associate to join dedicated Primary Care providers at PMG Sherwood Family Medicine clinic. You’ll be part of a collaborative, value-based, and patient-centered medical home made up of physicians, advanced practice providers, behavioral health providers, clinical pharmacist, case management, and an experienced clinical support team. Located 15 miles from downtown, this family-friendly suburb in the Tualatin Valley offers small town charm, fantastic schools, several protected parks and nature preserves, a short drive to the wineries of Newberg, while still easily accessible to all the city amenities available throughout Portland, Oregon.

• Full-time (up to 1.0 FTE)
• 4 to 5 days per week schedule, Monday to Friday
• Paneled, team-based care with direct and indirect patient care (In Basket management)
• 2-years of post-graduate professional experience in a primary care setting strongly preferred
