•  Large multi-specialty medical office located directly across from the hospital in Crestview.
•  One clinic is located in the heart of Baker, FL with a population of 6,000 plus permanent residents.
• DeFuniak Springs - 4,880 sq foot. Fully furnished, currently staffed by ARNP.

3 Family Medicine physicians currently in community.
