CentraCare Coordinated Care Clinic will provide care and management for high risk patients and heavy users of high cost services, as well as Correctional Care.  

• Outpatient Only
• Part-time preferred
• Interest in Mental Illness, Addiction, Primary Care
• Supervise APP team
• Convenient Lab, X-Ray and pharmacy
• EPIC is EMR
