Fairview Health Services is seeking a Geriatric Medicine (Family Medicine or Internal Medicine) Physician lead to join Fairview Geriatric Services.  The physician lead position is comprised of .7 FTE clinical and .3 FTE administrative duties in the leadership role.

Fairview Geriatric Services is a team of Geriatric Physicians and Nurse Practitioners who provide transitional care for post hospital patients and primary care medical services to those in LTC nursing home and assisted living facilities.

Practice Details:

Fairview Geriatric Services is part of the Fairview Medical Group. We have approximately 34 Geriatric Nurse Practitioners and 6 Physicians providing on-site care in over 40 facilities surrounding Fairview/M Health hospitals.

• Work in collaborative teams with NP and team coordinator to provide on-site care where people live. 
• Meet the unique and growing needs of complex, frail, and special needs patients in care communities.
• Provide regular on site patient care visits, seeing an average of 15 patients per day. 
• Physician/Provider Team accessibility via phone 24/7.
• Oversee and participate in the Health Care Home team.
• Adhere to the patient and family centered values of the Health Care Home.

The clinical lead is the accountable clinical leader of Fairview Geriatric Services with day to day responsibility including: clinical quality improvement, medical staff development, clinic/specialty performance, physician engagement, and system performance. Represents unit in pertinent internal and external meetings. Coordinates with site administrative team on related matters.

This position works closely with Clinic Administrator and assures that systems and processes that support the patient / physician and clinical encounter and the overall patient experience are performing at a high level of proficiency.

Jointly with other clinic providers and through Site and Network operating committees, ensures that community and Fairview Group Practice standards/guidelines of medical practice are maintained. Actively promotes the provision of quality health care and excellent patient service, to implement and execute system strategies within appropriate clinic units.

Supervises: Authorized practitioners and physicians with Fairview Geriatric Services. With clinic administrator is jointly accountable for day to day operational performance of clinical area.

Leadership Accountability: Manages a combination of clinical practice and leadership role. Expected to maintain an active clinical practice with Fairview Geriatric Services.  30% time will be spent in administrative and 70% of time will be clinical.

Position Qualifications:

Graduate of an accredited ACGME Family Medicine  or Internal Medicine residency training program and Board Certified/Board Prepared with the American Board of Medical Specialties or American Osteopathic Association. Geriatric Fellowship or CAQ in Geriatrics preferred but not required. Licensed or ability to obtain Minnesota Board of Medical Practice Licensure. Ability to obtain and /or maintain DEA certification for the State of Minnesota. Meet threshold criteria/qualifications for Credentialing and Privileges.
