Providence PACE is seeking a Per Diem physician to join its team in Portland, Oregon. You'll join an established, interdisciplinary team, providing older adults with a single, coordinated program of comprehensive care and social services. Be a part of an exceptional team of care givers who are passionate about the well-being of older adults.

• Per Diem role, requires a minimum commitment of 4 daytime shifts (8:00 a.m. to 5:00 p.m.) availability per month
• Outpatient primary care setting, home visits and visits in community-based settings
• Collaborate with physicians, nurse practitioners, nurse care managers, social workers, pharmacists, psychiatric/mental health clinicians, and other team members.
• Board-certification in Family Medicine or Internal Medicine is required
• Geriatric or Palliative Care fellowship training is preferred
• Join a team dedicated to Diversity, Equity, and Inclusion

Providence ElderPlace is a Program of All-inclusive Care for the Elderly (PACE), and is the first program of its kind in Oregon. With eight conveniently located centers in the Portland area, Providence ElderPlace provides and coordinates all necessary care services for seniors to remain active within their communities. The PACE model offers medical care, physical and occupational therapy, lab tests and procedures, home care services and more.
