Services is seeking a casual Palliative Care Physician, Family Medicine
Physician, or Internal Medicine Physician who has interest in Home Care and
Hospice to join our team.
Practice Details:

Fairview Home Care and Hospice serves the entire lifespan, infants to
elderly.

The team includes 3 partial FTE physicians and one pediatric specific provider who
provides care for our pediatric hospice patients..

Work in collaboration with the administrative and
interdisciplinary teams.

Serve as physician liaison with local medical
community.

Collaborate with a team of hospice physicians and
Nurse Practitioners to provide clinical care in a hospice environment.

Call is managed through highly skilled nursing
staff.

An
interested candidate must have experience in palliative and/or hospice care and
be willing to lead IDT's.

Position Qualifications:
-
Graduate of an accredited ACGME Family
Medicine or Internal Medicine residency training program and Board
Certified/Board Prepared with the American Board of Medical Specialties or
American Osteopathic Association in Family Medicine or Internal Medicine.

Licensed or ability to obtain Minnesota Board of
Medical Practice Licensure.

Ability to obtain and / or maintain DEA
certification for the State of Minnesota.

Meet threshold criteria/qualifications for
Credentialing and Privileges.
