We are seeking an experienced Psychiatric Nurse Practitioner to join our Integrated Primary Care Behavioral Health team at the Presbyterian Montgomery Primary Care Clinic. The Behavioral Health Clinician, as part of the primary care team, identifies, triages, and manages patients with behavioral health problems within the primary care setting within the scope of his or her advance practice licensure. In addition, this individual will provide skill training through patient education strategies and will develop specific behavioral health change plans for individual patients and behavioral health protocols for target populations. The Behavioral Health Clinician also serves as consultant resource to Primary Care Providers.
