Nurse Navigator- Cooley Dickinson Joint Center- Northampton, MA

The Joint Replacement Service Care Navigator establishes, promotes, and coordinates care mapping and delivery for joint replacement patients. The Care Navigator works collaboratively with members of the multidisciplinary care team, including, but not limited to, the orthopedics and sports medicine office, office and operating room schedulers, operating room team, inpatient nursing and therapy services, case management, post-acute providers, Visiting Nursing Association, physicians, APCs, patients, families, and payers to ensure that the care is safe and effectively coordinated along the continuum.

The primary focus of the Care Navigator is to engage and support joint replacement patients from the moment a decision is made for surgery through the pre-operative and post-operative experience to discharge and post discharge care and surveillance. In addition to engaging the patient, the Care Navigator engages care team members along the patients journey to ensure that care “hand-offs” are seamless and timely. The Care Navigator provides education and counseling to patients, families, and care givers to ensure that all aspects of the patient’s care plan are followed. The Care Navigator acts as liaison between the patient and the care team, bringing patients’ questions and concerns to the care team and conveying/reinforcing care team information with the patient. The Care Navigator is key in identifying patients at high risk for rehospitalization and/or potential for post-operative complication and engaging the team in developing strategy to mitigate these concerns.

This position reports to the Practice Administrator and operates within established organizational and departmental policies and procedures.

  • Associate degree in Nursing required; Bachelor’s Degree in Nursing and/or relevant experience required
  • A minimum of two (2) years of clinical experience in an ambulatory or inpatient hospital setting
  • Licensure in the Commonwealth of Massachusetts as a Registered Nurse required
  • Current BLS certification from the American Heart Association required
  • Demonstrated leadership skills, excellent interpersonal skills, and the ability to practice independently and as a team member required
  • Knowledge of basic computer skills required
  • Ability to maintain patient confidentiality required
  • Ability to communicate effectively with patients, peers, physicians, and family members required
